Support Coordination at Direct Focus Solutions

Direct Focus Solutions (DFS) can provide Support Coordination (Level 2 and 3) services for NDIS Participants within our service locations. Our support coordinators are able to provide comprehensive individually tailored supports for participants and help them to navigate their plans and achieve their goals with ease and confidence.

In addition to possessing a robust in-house team of clinicians and support workers, DFS works closely with external healthcare teams and service providers to secure the best possible services, resources, and community supports for NDIS participants. We pride ourselves on our ability to work alongside participants and their families and representatives to deliver solutions that encourage independence, achieve optimal results, and enhance their overall experience.

 

Support Coordinators provide three different levels of services. These are:

  • Level 1: Support Connection
  • Level 2: Support Coordination
  • Level 3: Specialist Support Coordination

Support coordination services at DFS are delivered independently from other services at our organisation, unless mutually determined by all parties to be suitable for the participant.

Generally, all support coordination services (across all levels) will feature one or more of the following key elements:

  • Providing guidance and information
  • Assessing participant needs and goals
  • Linking participants with available resources
  • Advocating for the individual
  • Coordinating services and supports
  • Making recommendations for support plans
  • Monitoring progress and outcomes

Choice and Control: Empowers participants to make their own decisions and achieve greater agency over their own lives and the services they will receive.

Access to Services: Links participants with the best services and supports available – supporting a comprehensive and holistic approach that takes into consideration all factors pertaining to the participant.

Improved Outcomes: Comprehensive review of goals and what services are needed to achieve these – maximising potential and helping participants live their best life.

Streamlined Service Delivery: Reduces stress for participants and their families/representatives and ensures a smoother, more hassle-free experience for all involved.

It is essential that NDIS participants are matched with a suitable support coordinator. We believe that participants should consider the following four qualities when choosing the right support coordinator for them:

  1. Do they understand your needs?
  2. Are they knowledgeable about the NDIS?
  3. Do they respect your individuality and choices?
  4. Are they able to communicate effectively?

Level 1 – Support Connection:

  • Helps participants understand and start applying their NDIS plan.
  • Builds participant skills to help them connect with providers and services (NDIS service providers, informal supports, and community supports available.)
  • Builds participant confidence in understanding and managing their plan.

Level 2 – Support Coordination:

  • Provides ongoing support to help participants manage engagements with multiple service providers.
  • Helps participants design their own supports, particularly when there are complex circumstances or multiple systems to navigate.
  • Develops a participant’s ability to direct their own lives and advocate for themselves to receive necessary supports and services.
  • Ensures that different services work well together.
  • Helps participants resolve issues and review their plan.

Level 3 – Specialist Support Coordination:

  • Specialist supports for participants with very complex support needs or who are at high risk (e.g. participants leaving the justice system or a mental health facility).
  • Applies a coordinator’s professional knowledge and experience in fields such as psychology, occupational therapy, social work, or mental health.
  • Helps participants and their families/representatives manage crises and build long-term stability.

This service focuses on supporting participants to take action to direct their own lives, not just to make choices related to their services. To achieve this, DFS assists participants to:

  • Build and maintain a resilient network of formal and informal supports
  • Receive coaching services as required
  • Develop their networking capacity and resilience

This process involves working together with the participant to better understand their available funding, identify their expectations from services, and collaborate to redesign the participant’s supports in a manner that best reflects their unique aims and goals.

This service helps participants to identify and address complex barriers that are impacting their ability to implement their plan and access appropriate supports. By working with Specialist Support Coordinators, participants will be able to:

  • Reduce the complexity of their support environment
  • Streamline the delivery of their supports

  • Advocacy and Rights – We ensure participants can access the full range of benefits available under their rights.

  • Crisis Intervention and Support – DFS can offer participants immediate assistance during critical times.

  • Victim Compensation Process – We support participants through the process of victim compensation claims, such as those who are survivors of domestic violence.

  • Assistance with NCAT Tribunal Matters – Participants can receive guidance throughout the complex tribunal processes for disputes or plan reviews.

We are seeking Support Coordinators!

DFS is currently looking to hire additional support coordinators to grow our multidisciplinary team and enhance services for our clients.

Interested applicants should ideally possess:

  • Prior work experience in NDIS Support Coordination or a similar role within a business environment.
  • Qualifications in a relevant allied health or community services field such as psychology, community services, or mental healthcare.
  • Ability to provide level 2 or 2 support coordination services for Participants.
  • Prior coordination experience working alongside people with a diverse array of disabilities and support needs.
  • An understanding of the disability and community sectors within the context of the NDIS.
  • Computer literacy and the ability to utilise hardware and software relevant to performing the role.
  • Administrative and organisational skills.
